What we do

COORDINARE commenced operations as the South Eastern NSW PHN in July 2015 and has a 50,000km footprint covering large areas of the Traditional Lands of the Yuin, Dharawal, Ngunnawal/Ngambri and the Ngarigo nations.

What is a Primary Health Network?

COORDINARE is one of 31 PHNs established across Australia by the Department of Health with the key objectives of increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes, and improving coordination of care to ensure patients receive the right care in the right place at the right time.

Our work

COORDINARE works directly with GPs, other primary health care providers, secondary health care providers and hospitals to improve and better coordinate care across the local health system, for patients requiring care from multiple providers or at risk of poor health outcomes. We find innovative ways of building ‘one coordinated and sustainable health system’ which ensures improved health outcomes, better consumer experiences, enhanced provider satisfaction and increased value for money.

COORDINARE is a not-for-profit organisation, with three members: University of Wollongong, Peoplecare and IRT. Our governance structure embeds strong links between the Board, our Councils and Committee, enabling a high degree of input into Board discussions. To deliver on these outcomes and key results, our highly skilled staff work across four key strategic approaches. These include:

  1. putting consumers front and centre of all we do.
  2. supporting general practice as the cornerstone of primary care.
  3. commissioning services for those most at risk of poor health outcomes.
  4. partnering to integrate services and systems.

This is all detailed in our business model which is data-driven, uses epidemiological information, practice-based evidence and local knowledge.
