Chronic conditions are the leading cause of illness, premature mortality, and health system utilisation in the South Eastern NSW region. We have an important role in facilitating, linking, and co-designing activities with health and social care partners to improve the prevention and management of chronic conditions.

    We work with primary care to create and promote new models of care and build the capacity of our region for self-management.

    Chronic conditions are long-term, complex, and can compromise quality of life. Improving care coordination for people ensures they receive the right care, in the right place, at the right time.

    We commission activities that reduce lifestyle risk factors, enhance the management of chronic conditions, and increase the efficiency and effectiveness of medical services for patients at risk of poor outcomes.

Improving care for people with chronic conditions

We aim to reduce potentially preventable hospitalisations due to timely and effective health care. We offer training, education, information, resources and business tools to encourage consistent, best-practice care for chronic conditions.
