Chronic conditions

The prevention and management of chronic conditions is a key priority for COORDINARE. Chronic conditions are the leading cause of illness, premature mortality, and health system utilisation in the South Eastern NSW region. 

Why is this an area of work?

Chronic conditions occur across the life course, often becoming more prominent with older age. Chronic conditions are long-term, complex, and can compromise quality of life. Improving care for people with chronic conditions can make a real difference to the long term health and wellbeing of local residents.

As the Primary Health Network for South Eastern NSW, COORDINARE has an important role in facilitating, linking, and co-designing activities within local primary health care, and across the broader health and social care sectors, to improve the prevention and management of chronic conditions.
