Models of care

In recognition of the growing demand on palliative care services within South Eastern NSW, and the critical role that primary care providers can play in supporting end of life and palliative care, COORDINARE commissioned the development of a Palliative and End of Life Model of Care for the Illawarra Shoalhaven and Southern NSW.

The models adopt a stepped care approach offering interventions that match an individual’s needs, ranging from the least to the most intensive. They allow an individual to access the right service to meet their needs at the right time, in the right place.

The models aim to:

  • support more consistent involvement of GPs in end of life and palliative care
  • strengthen the coordination and management of end of life care between GPs, the specialist palliative care service, primary health nurses, and care providers
  • improve outcomes for patients and their palliative care needs, as well as the needs of families and carers, including support for dying at a preferred place.
