Patient Centred Medical Home

The Patient Centred Medical Home (PCMH) model of care puts the patient at the centre of their care, helping to ensure each patient has the care team they need.

As the population aged over 65 steadily increases the frequency of health services being used will also increase. The PCMH model will help to address the change required in the health care system. Enhancing the role of nurses and other health professionals in providing more coordinated care to patients could reduce the demand for more general practitioners, and ensure patients are still receiving an appropriate level of care.
