In March 2019, COORDINARE – South Eastern NSW PHN invited Expressions of Interest (EOIs) from general practice to apply for funding to integrate pharmacists into the general practice care team. Four practices were successful in receiving funding in this first round. One practice achieved significant outcomes around opioid de-prescribing. The project has demonstrated key stewardship principles of interdisciplinary clinical governance and patient engagement that are transferable and able to be localised within other general practices.
Leveraging the stewardship principles and associated resources developed in the first round of Pharmacy in the Practice, this project seeks to undertake an extension, by scaling up this work.
COORDINARE is inviting EOIs from general practice to integrate pharmacists into their general practice care team. Practices will have the opportunity to focus on opioids or select a different medication(s) based on practice data and identified need.
