We commission to drive and implement reform, to position and arrange for system integration and invest in services that have a positive impact for the community. Through our commissioning, we collaborate, build strategic partnerships and make change for the better.
Co-design plays a role at every step of the commissioning cycle, throughout the strategic planning, procurement and evaluation phases. At each step of our commissioning approach there are opportunities to involve the community and key stakeholders to understand and address the needs of the region.
Understanding the health needs of communities, and how people prefer to use and access services, helps COORDINARE to design better connected models of healthcare, and to work with available service providers to meet consumer needs.

COORDINARE - South Eastern NSW PHN receives funding from the Commonwealth Department of Health and Aged Care to commission primary mental health services for people with severe mental illness who are being supported in primary care.
We are currently progressing a review of commissioned severe and complex primary mental health services to improve how these services can best meet the needs of people they aim to support.
The current program model was established in July 2017 and is delivered by two providers:
- Shoalhaven Family Medical Centres' Rural Outreach Mental Health Service (ROMHS)
- Grand Pacific Health's Integrated Recovery Service (IRS)
Regularly evaluating the effectiveness and suitability of the services that we commission is an important responsibility of COORDINARE towards our purpose of improving the health of communities in South Eastern NSW.
The information and insights gathered through this review will be considered and used by COORDINARE to inform our approach to future commissioning of these vital services for people in the South Eastern NSW catchment.
In early 2025, we are aiming to commence any change management processes required, to ensure certainty and continuity for existing clients and workforce involved in these programs.
Thank you to community members and providers who participated in recent consultations. We had high quality engagement and feedback across each of the sessions provided which included consultation feedback from more than 200 individuals.

We are considering and exploring the challenges and opportunities of Phase 2 key findings.
We anticipate that we will be able to provide formal advice regarding the outcome of the review in June 2025.

COORDINARE is commissioning the Healthy Hearts Healthy Minds program to address cardiovascular disease (CVD), a priority health condition in South Eastern NSW.
The program is being funding through the “PHN commissioning of Multidisciplinary teams” funding stream, announced as part of the Building a Stronger Medicare package in the 2023-24 budget.
Healthy Hearts Healthy Minds involves a nurse-led intervention for people who are at risk of CVD and is delivered in the general practice setting.
The program will initially involve nine general practices who will host a practice nurse employed and managed by an external provider.
This engagement aimed to test and refine the Healthy Hearts Healthy Minds service model by involving consumer representatives and general practices contracted to deliver the model.
The objectives of the engagement activities were to:

introduce the current ‘model of care’ for the Healthy Hearts Healthy Minds program and understand how it addresses the needs and preferences of the target audience

collect insights and suggestions that can strengthen the design and delivery of the program, relating to what the program offers, who it reaches, and how it is delivered.

analyse and summarise key considerations to inform ongoing commissioning and establishment of the program.
Thank you to consumer representatives, GP’s and other expert advisors who participated in recent consultations.
Our engagement included:

2 focus groups with consumer representatives (14 participants), as well as an online survey response from a consumer

1 focus group with participating practices/expert partners (8 participants).

The feedback generated from these engagements will be used to improve the model of care for the Healthy Hearts, Healthy Minds Program.
This service will be implemented from July 2025 until June 2027.
The program model will evolve over time in response to feedback from involved GP practices, practice nurses, and program participants.
An independent evaluation will be undertaken prior to the end of the program in 2027.
