Continuing to Be Me (C2bMe)
Continuing To Be Me (C2bMe) provides mental and emotional support to people living in Residential Aged Care Homes in Southeastern NSW.
Continuing to be Me (C2bMe) is funded by Coordinare and is a free therapeutic mental health program designed to support eligible people living in Residential Aged Care Homes (RACHs) in Southeastern NSW who are presenting with mild to moderate mental health distress.
C2bMe provides person-centred and evidence-based 1:1 mental health support and group programs for eligible residents who reside in RACHs partnered with the C2bMe program.
C2bMe seeks to holistically support residents’ mental health by offering capacity building and training opportunities to Care Staff to support their understanding of mental health with their residents.
C2bMe is designed to:
- Provide a comprehensive person-centred model of care.
- Reinforce the person’s identity and purpose.
- Increases connectedness and hope.
- Improve the person’s mental health wellbeing.
- Increase the day-to-day functioning of residents in aged care homes.
C2bMe is a free service for people who are:
- Residents of a partnered Aged Care Home, within the Southeastern NSW area
- Aged 65 years or older, or Aboriginal and Torres Strait Islander people aged 50 or over
- Experiencing mild to moderate mental health concerns or at risk of developing mental health concerns
- Consent to receive support
- Able to actively participate in mental health support, this requires them to have the capacity to reflect and retain information.
