Eden partners unite behind priorities for better local healthcare

The Eden Collaborative Care Project partners have agreed on priorities to address some of the community's most pressing health challenges. The project, which began in May 2025, brings together health services, community organisations and government agencies to take a place-based approach to healthcare planning in Eden.
The launch of the Small Grants Initiative in Eden for health and wellbeing is a tangible step to implement these priorities. Opening on 13 July, grants of $500 to $5,000 are available to local community groups and organisation for activities that help improve people's health and wellbeing. A detailed picture of local health needs was developed through a community survey completed by 267 residents, health professional interviews and data analysis. The collaborative process culminated in a series of workshops where partners agreed on three priority areas for action:
- Improving service integration and investigating co-location opportunities.
- Promoting preventive health and community wellbeing.
- Exploring solutions to address transport barriers and improve access to care.
COORDINARE – South Eastern NSW Primary Health Network is proud to have been a partner in helping to identify solutions. These include new care navigation services to help locals access the right care, promotion of community grants that support health and wellbeing, joint health education activities, and the introduction of supported telehealth locations throughout the town.
On 7 August, COORDINARE is excited to be hosting the Health Check Convoy in Eden in partnership with local providers to deliver free, accessible health and wellbeing services. All services are available between 10am - 3pm with no appointments needed. Learn more about the Health Check Convoy here.
The Eden Collaborative Care Project demonstrates the value of local organisations working together to understand community needs, coordinate services and identify solutions that will make healthcare easier to access and navigate for local residents.
The program is supported by the NSW Ministry of Health and facilitated by RDN Health. It focuses on helping rural and regional communities access the right health services so people can get healthy and stay healthy.
