Medically supervised, home-based withdrawal and recovery support for individuals looking to change their relationship with alcohol, cannabis, or stimulants.
Commonwealth Psychosocial Support Program
The program provides FREE, short-term support for people aged 16 and over who are living with severe mental health challenges.

Care Finder
Care Finder is a free service that can help vulnerable older people in South Eastern NSW who need extra, intensive support to access aged care services and other supports in the community.
The service can assist with tasks like completing forms and understanding aged care service agreements, talking to My Aged Care, arranging, attending and providing support at an assessment plus finding and short-listing local aged care providers.
Care finders can also assist older people in need to access health, mental health, housing and homelessness, drug and alcohol services and community groups.
COORDINARE has funded organisations in South Eastern NSW to link eligible community members with care finders.

Consumers aged 65+ / or 50+ for Aboriginal people or those experiencing homelessness or low income. Depends on the consumer having 1 or more of the following indicators:
- have difficulty communicating because of language or literacy problems
- find it difficult to understand information and make decisions
- be reluctant to engage with aged care or government
- be in an unsafe situation if they do not receive services.

Gateway Life are offering free access to an 8-week gym-based program combining exercise and education for people aged 45 and over who are at risk of developing chronic conditions. Participants will attend weekly group sessions, alternating between physical activity and education.

Clean Slate Clinic
Clean Slate Clinic is a virtual healthcare service in Australia, offering medically supervised, home-based withdrawal and recovery support for individuals looking to change their relationship with alcohol, cannabis, or stimulants.
By delivering care via telehealth, Clean Slate removes traditional barriers to detox - enabling people to safely begin their recovery journey from the comfort and privacy of their own homes.
This 12-month program supports you through withdrawal, detox and recovery by providing personalised planning, medical support, regular follow-up, peer support, and relapse prevention strategies.
The program also includes access to specialist doctors and nurses, medications where needed, a tailored recovery plan, and ongoing support after detox. With your consent, your regular GP will also be kept informed so they can help support your recovery journey.
The model is flexible and responsive, designed to meet each person’s goals and level of readiness for change.

The Commonwealth Psychosocial Support (CPS) Program provides FREE, short-term support for people aged 16 and over who are living with severe mental health challenges.
The program helps you work towards personal recovery goals, improve wellbeing, build social connections, and access the services and supports you need.
Support may include one-on-one peer support, group activities, service navigation, and assistance with NDIS applications if appropriate.

Continuing to be Me (C2bMe) is a FREE mental health and wellbeing program for older people living in participating residential aged care homes.
The program provides one-on-one support and group sessions with mental health professionals to help residents improve their wellbeing, build social connections, maintain a sense of purpose, and enhance their quality of life.
C2bMe also works closely with families, GPs and aged care staff to ensure residents receive personalised, holistic support.
