The Aged Care Assessment Services determine your eligibility for subsidised aged care. A two-part assessment process is undertaken to gain an understanding of what services are best suited to your needs. An eligibility check is conducted online or over the phone, followed by an in-person assessment.
To see if you are eligible, click here: My Aged Care - Am I Eligible, or call My Aged Care on 1800 200 422. Once your eligibility is confirmed, you can obtain a referral to your local hospital's assessment service.
