Our role and region
COORDINARE is the South Eastern NSW Primary Health Network (PHN). Our role is to improve the health and wellbeing of our community which is one of the largest rural and regional populations in NSW.
Our region stretches from Helensburgh in the north to the Victorian border in the south and inland to Cooma / Monaro, Queanbeyan, Yass and Goulburn.

COPD Action Plan reduces fear and hospital visits for Yvonne

Yvonne Kelly says she has managed to avoid ED four times since her COPD Action Plan was developed. A COPD Action Plan is a simple guide that helps people manage symptoms of COPD (chronic obstructive pulmonary disease), including flare ups.
