The COORDINARE team consists of highly skilled staff who are committed to working with general practice and other stakeholders to enhance the coordination of care, improve health outcomes and provide a better patient experience.
Every general practice in South Eastern NSW has a Health Coordination Consultant (HCC) from COORDINARE. They work collaboratively with stakeholders within the community to facilitate improvements through health system redesign.
