Social RX expanded to provide support to more patients

After the successful launch of Primary and Community Care Service’s (PCCS) Social Rx social prescribing services in the Illawarra and Shoalhaven region in 2021, COORDINARE is excited to see this service expand across the catchment, and now be accessible to residents in Southern NSW.
Social prescribing aims to support patients to tackle the non-medical barriers to health and wellbeing including loneliness, isolation, housing, financial, and other socioeconomic factors.
Social Rx compliments the recently launched Chronic Care Coordination program, with both programs providing additional resources to general practice to help patients manage their complex health needs and navigate an often confusing network of health and social services.
A range of services are now available that provide patient support depending on their health needs, consider the following service options and if your patients might benefit from a referral:
- Social Rx is available for people aged 18 years and over at risk of developing or have a long-term health condition or need practical health with social issues.
- If your patient needs help with both social issues and their chronic health condition, consider referring to the Chronic Care Coordination program:
- Aboriginal patients may wish to be referred to the Integrated Team Care program for care coordination, available to those who have 1 or more chronic condition (Diabetes, CHD, Lung disease, Renal disease or Cancer).
