EOI Grant Opportunity - Connect Well Outreach Healthcare for Victim-Survivors of Family, Domestic and Sexual Violence

Background

COORDINARE - South Eastern NSW Primary Health Network (SENSW PHN) invites expressions of interest from general practices wishing to nominate female GPs to participate in the Connect Well program, operating within crisis accommodation services in three locations in South Eastern NSW. These locations are:  

  • Moruya   
  • Goulburn   
  • Nowra  

The project will run for two years from July 2025 - June 2027.  

Connect Well is a comprehensive and integrated model designed to meet the health needs of victim-survivors of family, domestic and sexual violence (FDSV) who have experienced gaps in health care because of experiencing and fleeing FDSV.

The program is centred around the development of a dedicated Primary Healthcare Outreach Team (PHOT) which includes a GP, nurse and a health linker position, who work together as a multidisciplinary team to deliver the program at participating crisis accommodation services. This funding opportunity is for the GP as part of the PHOT.   

The PHOT will collaborate to provide medical treatment and care coordination. Where there are barriers to access to health care services, brokerage funding will be available for Connect Well clients (women, infants and children).   

General practices in the Eurobodalla, Shoalhaven or Goulburn-Mulwaree local government areas are eligible to apply for funding and nominate a female GP for this program. There will be an opportunity for the GPs to engage in funded training prior to commencing work for this service.
