Coordinating Care for Culturally Diverse Communities

Background
COORDINARE is inviting expressions of interest (EOIs) from suitable organisations to apply for funding to support existing local services to develop and pilot an initiative to increase their capacity to support people from culturally and linguistically diverse communities to engage with health and community services.
Through this funding, COORDINARE seeks to commission a care coordination model for people from culturally diverse backgrounds, that enables navigation of the complex health system, coordinates complex health needs, identifies and addresses barriers to heath care and empowers the community in self-management of their health conditions.
A total of $466,997 ex GST is available for this initiative. Funded projects will run for 12 months from date of execution.
All activities funded must occur within, and target current residents of the South Eastern NSW region. Activities may occur within a small or large area within the region if evidence of priority target groups/areas are outlined in proposals. All projects will be subject to an evaluation.

Eligibility
Organisations will not be eligible for direct funding if they are:
- General Practices, or
- Organisations not currently delivering services in SENSW region.

Applications close at 5pm AEDT 23 April 2025.
To apply complete all sections of the EOI electronic form on TenderLink (registration is free) and include all relevant documentation as outlined in Section 10 of the EOI guideline under Assessment Criteria.
Submitting an incomplete application may disqualify you from the evaluation process.
