Background
Practice nurses collaborate closely with GPs and other healthcare professionals within the practice team, contributing to a multidisciplinary approach to patient care. By leveraging the expertise of practice nurses, general practitioners can optimise their workflow, maximise their scope of practice, and deliver high quality, patient-centred care.
COORDINARE - South Eastern NSW PHN is seeking to commission small general practices to enhance clinical care for patients with hypertension and hyperlipidaemia through the provision of a Practice Nurse who will deliver a multidisciplinary model of care.
Grant details
COORDINARE - South Eastern NSW PHN is offering grants to small general practices which are defined as those practices who have three (3) or fewer FTE general practitioners. Successful practices will receive a set up grant payment of $15,000 (ex-GST) in addition to the participation of the part-time Practice Nurse within the practice.
There are up to ten (10) individual grants available. Funded projects will run until 31 December 2027. The one-off $15,000 grant (ex-GST) can be used to support the costs associated with setting up systems, processes and a suitable workspace for the Practice Nurse to enable them to undertake the requirements of the care pathway.
