Older adults primary care capacity building program

Background

COORDINARE is seeking interest from General Practices to participate in an initiative aimed at enhancing the skills, confidence, and experience of the primary care nursing workforce.

The program aims to enhance the skills, confidence, and experience of the primary care nursing workforce. Helping to support healthy ageing in South Eastern NSW and improve the care provided to older adults in the community. 

The program also seeks to build practice capacity to better support healthy ageing and the care of older adults within the community, reducing the need for more complex services and/or early entry into residential aged care. 

The program includes:

  • Grant Package of $10,000 to support project activities over a 12-month period until July 2026.
  • Funded nurse position in the Gerontological Nursing Competencies (GNC) program enhancing your practice’s expertise in the care of older adults.
  • Access to a Practice Nurse Community of practice for ongoing mentoring and support.
