    Population Health Profile

    To support our approach to population health planning, we analyse and interpret a wide range of data, topics and variables. This forms our Population Health Profile, which shows the health and social profile of our region.

    Insights from this are then translated with the needs of community, stakeholders and health professionals to develop our Needs Assessment.

    The Population Health Profile is updated on a regular basis.

    Care Finders Needs Assessment

    This supplementary needs assessment is a summary of all local needs identified in South Eastern NSW, in relation to care finder support.

    It continues our comprehensive population health needs analysis and service gaps assessment, focusing on the wider topic of healthy ageing and also most of the specific topics around planning for the Care Finder service.

    Needs Assessment

    Our Needs Assessment is a comprehensive population health needs analysis and service gaps assessment.

    It incorporates a refreshed approach to review and incorporate the latest quantitative data from several topics and sources, alongside an analysis of qualitative data collated and sourced from ongoing stakeholder consultation, community and consumer inputs.

    Innovate Reconciliation Action Plan

    Our Innovate Reconciliation Action Plan (RAP) builds on the learnings of our Reflect RAP, deepening our understanding of the reconciliation process, cultivating relationships, and identifying opportunities to gain better understanding.

    It is our commitment to the communities and organisations we work with to be truthful in our work together with a vision to progress and celebrate health, well-being and equity for and with First Nations peoples.

    Mental Health Lived Experience (Peer) Work Framework

    Our Mental Health Lived Experience (Peer) Work Framework was designed and developed with Southern NSW and Illawarra Shoalhaven Local Health Districts, community-based organisations, and people with lived experience of mental illness who also work in mental health services.

    The framework ensures people with experience of mental illness continue to shape and deliver mental health services.

    February 2021

    Regional Mental Health and Suicide Prevention Plan

    We worked collaboratively with the Illawarra Shoalhaven and Southern NSW Local Health Districts to develop a Regional Mental Health and Suicide Prevention Plan.

    The plan has nine priority areas and involves working together across the health system, with consumers and other stakeholders, to provide better integrated care for people with mental illness.

    Southern NSW Integrated Care Strategy

    The Southern NSW Integrated Care Strategy outlines the commitment of COORDINARE and Southern NSW Local Health District to work collaboratively to integrate healthcare for residents living in Southern NSW.

    Integrated care is a collaborative approach to strengthen person-centred health care delivery through the provision of coordinated, high quality care across the lifespan.

    Reflect Reconciliation Action Plan

    Our Reflect Reconciliation Action Plan (RAP) aims to develop tangible goals that we can implement over time to foster healing and recognition for Aboriginal and Torres Straight Islander peoples in our region.

    The RAP will provide us with a set of priorities and guidelines to help us engage more meaningfully with Aboriginal and Torres Strait Islander communities, histories and cultures.

    Illawarra Shoalhaven Integrated Care Strategy

    We work with local health providers to ensure greater service and system integration between primary and acute care.

    To ensure care is seamless, timely and appropriate, we have built a strategic alliance with the Illawarra Shoalhaven Local Health District, and developed the Board approved Integrated Care Strategy.

    November 2017

    Strategic Plan 2017-2020

    Our Strategic Plan 2017-2020 was developed in consultation with our stakeholders and staff, and workshopped intensively with our governance groups at a joint meeting in September 2017.

    The input of our Clinical Councils and Community Advisory Committee members ensured that we have a strong plan with clear strategic and health priorities through to 2020.
