Background
COORDINARE - South Eastern NSW PHN invites applications from general practices and Aboriginal Medical Services participating in the Workforce Incentive Program – Practice Stream to develop a program for multidisciplinary team-based learning in their practice.
Multidisciplinary learning within primary care promotes development of the skills and knowledge required to deliver high quality patient care. This funding opportunity will support general practice and Aboriginal Medical Services to enhance existing models of care through interprofessional education and capacity building at the practice level.
There is funding of up to $6,000 (excl. GST) per practice, available for 15 practices, commencing from June 2025 to 30 November 2025.
Eligible practices are those practices within the SENSW PHN catchment who are registered and participating in the Department of Health and Aged Care’s Workforce Incentive Program (WIP) – Practice Stream (PS) incentive.

- All compliance documents as outlined in the application form.
Email the completed documents to commissioning@coordinare.org.au
Note: Practices must adhere to output and outcomes measurement as outlined in Attachment 2. Practices will also be required to provide a short final report at the end of the project. Reporting templates will be provided by COORDINARE.

FAQs
Will there be a Q & A sheet or similar record of any questions that may come in and will it be made available?
Yes, individuals and practices will have the opportunity to ask questions up until 5pm Wednesday 14 May 2025. Responses to all questions will be uploaded to COORDINARE’s website by COB Thursday 15 May 2025
If a practice is successful in receiving the funding, does the funding have to be used to deliver training to their own practice staff specifically or can they invite neighbouring practices to participate?
Although the intent of the funding was for practices to build interprofessional collaboration amongst themselves, COORDINARE have no objections to practices inviting neighbouring practices to participate if that is something they would like to consider.
