Your Primary Health Network

COORDINARE is the South Eastern NSW Primary Health Network (PHN). We are uniquely positioned to support AHPs to better integrate into the primary health care system, through our understanding of community health needs and the broader health care system.

Together, closer engagement between PHNs and AHPs can promote the value of integrated models of care, with a focus on prevention and early intervention to improve patient outcomes and the cost effectiveness of health services.
