Important Update: COORDINARE and Illawarra Shoalhaven Local Health District Fast Forms

To improve the referral experience of patients, referrers and clinicians in outpatient services, NSW Health is currently developing a new statewide digital referral management solution.

This new system is due for rollout in early 2024 and will enable referrals to be sent electronically to outpatient services and notifies referrers and patients of referral outcomes in a timely manner.

It features electronic secure messaging, improving communication between clinicians, referrers, administrative staff and patients, and enhancing the security of sensitive patient information. More information is available here.

In order to prepare for the rollout of the new system, we are required to retire the local electronic referral system ‘Fast Forms’ on 1 December 2023. As an interim measure, we encourage you to send any referrals to Illawarra Shoalhaven Local Health District (ISLHD) Outpatient services via fax. Details for individual services can be found below.

Your valued participation in Fast Forms developed in partnership with the ISLHD, has been used to inform the statewide program. However, we understand this change may affect your current workflow and we endeavour to support you during this transition period.
